Benefit Overview

• Routine hearing exams covered through participating TruHearing providers.
• Plan provides an annual allowance toward the purchase of hearing aids.
• Member cost-share varies based on the hearing aid technology selected.

Hearing Aid Purchase Includes

• Annual hearing exam at no charge
• 3-year comprehensive warranty (loss, damage and repair)
• 2-year battery supply (up to 64 batteries per hearing aid per year)
• 60-day evaluation period
• 1-year follow-up service at no charge with the original provider
• Professional services including testing, fitting, programming and support

Important Reminders

• Members must contact TruHearing to schedule appointments with participating providers.
• TruHearing manages the process from scheduling through hearing aid purchase.
• Members are responsible for any amount exceeding their annual hearing aid allowance.
• Out-of-network hearing aid purchases are not covered.
• Member reimbursement forms for out-of-network services will not be accepted.
• Benefits and allowance amounts vary by plan.
