Cardiovascular services: Helping you keep your heart healthy

Managing your risk factors for cardiovascular disease is essential to your heart health — and it’s a focus of Prominence Medicare Advantage. We make it easy to connect with providers and services committed to meeting your individual healthy heart needs at little or no cost to you.

Cardiac Plan

In Washoe county and Texas we’ve developed a plan especially for people with cardiac conditions. If you have heart disease or other related conditions, you may qualify for our new Heart Plan (in Washoe) or our Diabetes & Heart Plan (in Texas), which provide specialized care for your cardiovascular or heart condition(s). Plus, some specialists, including cardiologists, are available for $0.

Qualifying for the Prominence Heart Plan is simple:

  • Step 1: Your physician has told you that you have one or more of the following cardiovascular and/or heart conditions: cardiac arrhythmias, coronary artery disease, peripheral vascular disease, chronic venous thromboembolic disorder or congestive heart failure.
  • Step 2: Work with your insurance broker or call us at the number above to submit the application to enroll in the Prominence Heart or Diabetes and Heart Plan.

With Prominence Medicare Advantage, you get:

  • Access to a network of caring and skilled local providers to help stay on track and manage key heart-health indicators like blood pressure and cholesterol;
  • Local events and educational opportunities to help you make the most of your benefits and take care of your health;
  • A free Prominence Health Assessment (PHA) to develop your own customized preventive care plan—plus you get a gift card just for completing the assessment! This one-hour assessment provides an opportunity to review your medical and family history, run a series of useful tests and develop a set of personalized next steps just for you.

Keeping diabetes in check? We’ve got you covered

With diabetic supplies at a $0 copay for many medications and insulin products, Prominence Medicare Advantage is designed to help you more easily manage your diabetes, rather than diabetes managing you.

With Prominence Medicare Advantage:

  • Diabetic supplies, including Contour (by the manufacture Ascenscia) meters and test strips, are covered at a $0 copay.
  • Select brand-name insulins are covered at a lower copayment and never cost more than $35.
  • A 100-day prescription option for many medications reduces the number of copays, if any, to save you money on refills.

Additionally, a free Prominence Health Assessment (PHA) helps create a personalized preventive care plan. This once-a-year assessment is another way to help ensure you get recommended services and take advantage of opportunities to maintain and improve your health.

Of course, your doctor is your key partner and a great resource. Through regular doctor visits, you can get help staying on track with things like diet, exercise, medications and overall health concerns.

If you live in Texas and have diabetes, you may qualify for our new our Diabetes & Heart Plan, which provide specialized care for your diabetes. Plus, we have even more insulins available at a low or no cost. And, some specialists, are available for $0.
